Colovesical Fistula

OVERVIEW


Colovesical fistula (CVF) is a rare condition which is characterized by an abnormal open connection between the colon (large intestine) and the urinary bladder the organ which lies just below the colon. Due to this condition, fecal matter from the colon enters the bladder, and can cause potential health complications and painful infections. Colovesical fistula is a very painful condition. The formation of Colovesical fistula which is also called Vesicocolic fistula generally results from a surgical procedure or other trauma caused to the thick wall of tissue separating the colon and the urinary bladder.


The occurrence of Colovesical fistula is usually a complication which can result from a number of diseases. The diseases which can potentially trigger the fistula include diverticular disease, Crohn’s disease or some other inflammatory bowel diseases, colorectal cancers, (chiefly recto-sigmoid cancers), as well as gynaecological and urological cancers.


Besides the mentioned diseases which can cause Colovesical fistula, the condition can sometimes be also caused by presence of foreign bodies in the gastrointestinal tract, laparoscopic and open inguinal hernia repair, open surgical procedures involving the colon or urinary bladder, radiotherapy, benign gastrointestinal diseases, benign urological diseases, and cancer of organs surrounding the colon and bladder. Colovesical fistula is an uncommon condition, but it can be treated. However, the condition can have effect the sufferer’s quality of life, lead to significant morbidity and, possibly, even to death.


SYMPTOMS


Some of the common symptoms which are believed to be associated with the possible occurrence of Colovesical fistula are:

  • Abdominal cramps/pain

  • Diarrhoea

  • Irritative symptoms in affected areas

  • Passage of urine per rectum

  • Mixture of fecal matter in urine (leading to brownish-coloured urine)

  • Mixture of gas from the colon with urine (leading to bubbles in urine)

  • Painful or burning sensation during urination

  • Traces of blood in the urine.

TREATMENT


The treatment of Colovesical fistula generally involves the use of some conservative methods, if the fistula is fairly small. These conservative methods may include: use of antibiotics and steroid medications, resting the intestines by feeding the patients through veins, insertion of catheter into the bladder for draining out the seeped fluid from the colon.


Conservative methods of treatment of Colovesical fistula may help the fistula to close and heal by itself. However, in cases in which conservative treatment may not yield the desired result, surgery may be required to treat Colovesical fistula.
